Registration Procedure


1. Form(A) is only for [Long Island], [Manhattan] & [Flushing] locations,
2. Please refer to www.unitedaquatics.org to sign-up [Brooklyn] locations,
3. Each form is up to 3 students per family,
4. Fill in all information required for both student and the guardian,
5. Select the location, schedule, and the type of sport(s) that you are going to sign up,
6. Fill in emergency contact information,
7. Print-out and sign both of the "Student Waiver Form" & "Health Record Form",
8. A Full Payment with ($20 Registration Fee for new student and previous student who did not sign up for more than 1 year) must be mailed with the registration form before 09/22/10 payable to:
