Registration Procedure

1. Form(A) is only for [Long Island], [Manhattan] & [Flushing] locations,
2. Please use Form(B) to sign-up [Brooklyn] locations,
3. Each form is up to 3 students per family,
4. Fill in all information required for both student and the guardian,
5. Select the location, schedule, and the type of sport(s) that you are going to sign up,
6. Fill in emergency contact information,
7. Print-out and sign both of the "Student Waiver Form" & "Health Record Form",
8. A Full Payment with ($20 Registration Fee for new student and previous student who did not sign up for more than 1 year) must be mailed with the registration form before 12/29/08 payable to:

9. Our administrative staffs will contact you by phone when we have received your application

** Deposit is part of tuition. Balance must be paid by the first day of class.
